The Chronic Disease Management Plan (previously referred to as the Enhanced Primary Care (EPC) Plan) entitles patients to up to 5 sessions with a Speech Pathologist with a report (per calendar year) that are partially covered by Medicare. A General Practitioner is responsible for determining whether an individual is eligible to receive a CDM Plan. Each of the 5 CDM sessions will be partially covered ($52.95) from Medicare but full payment is required first then the Medicare rebate will be paid to you.

If you are paying privately and you do not have a Medicare Chronic Disease Management Plan (CDM) then you can use your health fund for the payment but all depends on your cover. The health fund might only pay a portion of the fee and then you will still have to pay the out of pocket fee.
